3. Isi juga Nama Negara asalmu“Country of Incorporation or organization“.
4. Centang “Individual” for “Type of beneficial owner“.
5. Isi Alamat Lengkap Rumahmu “Permanent residence address“, “City or town, state or province“, and “Country“.
6. Pada “Bagian 2: Claim of Tax Treaty Benefits“, Centang “a. The beneficial owner is a resident of [INDONESIA] within the meaning of the income tax treaty between the United States and that country.“.
7. Centang pada “Bagian 3: Notional Principal Contracts“.
8. Pada “Bagian 4: Certification“, Isi Nama Lengkapmu “Signature“.
9. Isi “owner” untuk “Capacity in which acting“.
10. Click “Submit information” button and you are done!
